We hereby release the ASMBA from any and all responsibility/liability for injury to members of this family incurred during games or practices and for any loss or damage to personal property. We further release the ASMBA and each and every one of its members from any and all claims that may arise either directly or indirectly as a result of joining the said Association.
I hereby give my permission to the ASMBA to photograph and/or videotape me or my family members and further use my name, face, likeness, voice and appearance in connection with exhibitions, publicity, advertising and promotional materials without reservation or limitation. The ASMBA is under no obligation to exercise said rights herein granted.
